The Community Health Network is seeking a dedicated and compassionate Clinical Social Worker (LCSW) for a full-time remote position based at our Hospice Center in Tampa, Florida. This role is critical in providing emotional and psychological support to our clients and their families during one of the most challenging phases of life. Key Responsibilities\n

• Conduct comprehensive assessments to evaluate clients' social, emotional, and psychological needs, tailoring interventions to improve their overall well-being.

• Provide both individual and group counseling utilizing various therapeutic methods, including cognitive-behavioral therapy and mindfulness techniques, to respond to diverse client needs.

• Develop and implement personalized service plans that align with the clients’ goals, enhancing their capabilities and facilitating positive change in their lives.

• Advocate vigorously for clients’ rights and access to essential resources, while working on broader social justice issues that affect their well-being.

• Engage in program development, including evaluating existing services and participating in grant reporting efforts to secure funding for essential initiatives.

• Collaborate with community organizations, healthcare providers, and government agencies to ensure clients receive holistic care and support.

• Partake in team meetings, training sessions, and community outreach activities to stay informed and contribute to the organization’s mission.

Patient Population / Client Focus\n

You will primarily engage with clients facing terminal illnesses and their families, providing compassionate end-of-life care and emotional support. Your psychosocial interventions will help clients navigate grief, loss, and adjustment issues while fostering a sense of peace and dignity during their palliative care journey.

Qualifications\n

• Master's degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) .

• Demonstrated understanding of trauma-informed care and its application in a hospice setting.

• Strong communication, empathy, and organizational skills to effectively coordinate care and collaborate with a multidisciplinary team.

• Previous experience within a nonprofit organization or healthcare setting, ideally in hospice or palliative care.

Compensation and Benefits\n
