Work Environment\n

This position allows for a remote work environment, providing flexibility while enabling you to effectively perform your essential functions from a distance. Interaction with clients is frequent via telehealth channels and scheduled virtual meetings, as well as necessary documentation and case management tasks. While the role is primarily remote, you may occasionally need to visit our Kansas City hospice center for team meetings or training sessions. Key Responsibilities\n

• Conduct comprehensive assessments and develop individualized treatment plans based on the unique needs of each client.

• Facilitate individual, family, and group therapy sessions, effectively addressing issues related to grief, loss, and life transitions.

• Monitor and evaluate client progress toward achieving treatment goals, adapting strategies as needed based on ongoing assessments.

• Collaborate closely with interdisciplinary teams, including medical staff such as nurses and physicians, to ensure holistic care.

• Provide crisis intervention for clients experiencing acute emotional distress, utilizing evidence-based strategies and support frameworks.

• Maintain detailed and accurate electronic health records of client interactions, ensuring confidentiality and compliance with HIPAA regulations.

• Stay informed about developments in mental health treatment methodologies, especially those pertinent to hospice care, through continuing education and professional development.

Patient Population / Client Focus\n

You will be working primarily with patients facing terminal illnesses, as well as their families. Clients may include those dealing with complex emotional challenges related to grief and loss, or exploring advance care planning. The provision of psychosocial support is pivotal, as you help clients navigate their feelings while fostering a sense of dignity during hospice care.

Qualifications\n

• A Master's degree in Social Work from an accredited program is mandatory.

• A current and valid state license to practice clinical social work is required.
\n
• Demonstrated ability to manage sensitive and confidential information with the highest degree of professionalism.

• Strong interpersonal and communication skills, both verbal and written, are essential for effective client interaction and collaboration with team members.
