As a remote position, our LCSW will enjoy the flexibility of working from home while also being part of a supportive and collaborative team. Our Hospice Center offers a unique setting focused on children’s services, where the LCSW will engage with other healthcare professionals to provide holistic care and support. Key Responsibilities\n

• Conduct comprehensive evaluations and assessments for infants and toddlers to determine their individual developmental needs.
\n
• Actively participate in the development and implementation of Individualized Family Service Plans (IFSPs) to tailor services based on child and family needs.
\n
• Monitor the progress and outcomes of the implemented plans as a proactive member of an interdisciplinary team.
\n
• Provide vital support and counseling to families, equipping them with the knowledge, skills, and resources to enhance their child's development.
\n
• Ensure compliance with all relevant federal and state regulations to maintain the integrity of services provided.
\n
• Foster a supportive and engaging relationship with children and families to create a positive learning environment.
\n
• Utilize evidence-based practices and Natural Learning Environment Practices in recommending strategies that align with the family's priorities.
\n
• Implement strategies that enhance child development through coaching and natural environment teaching.
\n
• Maintain adherence to safety and health standards to support the well-being of all staff and clients.

Patient Population / Client Focus\n

This role focuses primarily on infants and toddlers within the early intervention spectrum who face disabilities or developmental delays. The LCSW will work directly with families from diverse backgrounds, tailoring interventions to meet unique individual and cultural needs. A strong commitment to understanding the family dynamics and promoting a child's development in a nurturing, family-centered environment is essential to success in this role.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• Possession of a valid LCSW license issued by the Department of Public Health.

Compensation and Benefits\n
