Key Responsibilities\n

• Conduct and lead therapy sessions focused on psychosocial aspects of patient care, ensuring patients receive the emotional and therapeutic support they require.

• Facilitate psychoeducational groups that encourage participation and learning among patients, fostering a supportive atmosphere for shared experiences.

• Conduct clinical assessments to evaluate patient needs and formulate individualized care plans in collaboration with the healthcare team.

• Actively engage with the Utilization Review Department and Business Department to address authorization and payment processes, ensuring a seamless administrative experience for patients.

• Begin discharge planning on the day of admission, outlining key resources and support systems to assist patients in their transition home or to other care settings.
\n
• Complete discharge sessions with patients, confirming they understand their coping strategies, transition plans, and available resources.
\n
• Ensure all discharge appointments are confirmed with patients and their loved ones, facilitating clear communication before discharge.
\n
• Maintain ongoing communication with referral sources and outpatient providers to establish continuity of care throughout the patient's journey.

Patient Population / Client Focus\n

The primary focus of this role is on patients within a hospice center, dealing with serious and terminal illnesses. You will work with a diverse group of individuals and their families, providing emotional support, therapeutic interventions, and resources to enhance their quality of life. • Master’s degree in Social Work (MSW) is required.

Compensation and Benefits\n

The salary for this full-time position ranges from USD 120,000 to 130,000 annually , commensurate with experience and caseload responsibilities.
