Key Responsibilities\n

• Collaborate with healthcare team members to develop comprehensive treatment and discharge plans based on thorough psychosocial assessments.

• Conduct regular assessments and implement targeted interventions, tailoring approaches to meet the specific needs of each patient.

• Maintain precise and timely documentation, including clinical notes, progress reports, and other vital records, in line with established policies.

• Provide counseling and crisis intervention to patients experiencing emotional and behavioral challenges, helping them navigate complex feelings during their care.

• Engage with family members and support systems to discuss treatment options and assist in developing effective discharge plans that support continued care.

Patient Population / Client Focus\n

This position will primarily focus on patients in hospice care, who are coping with life-limiting illnesses. You will encounter a diverse population that requires sensitivity, patience, and strong clinical skills to address complex emotional responses. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.

Compensation and Benefits\n
