Work Environment

This full-time position is purely onsite, focusing on providing in-home care to patients across Newark and surrounding areas. As a Home Care Physical Therapist, you will work independently within patients' homes, allowing for an intimate understanding of their living environments and how it impacts their recovery. You will have the flexibility to choose your own hours during the standard work week (Monday to Friday), facilitating a balance that meets your professional and personal life.

Key Responsibilities

• Perform thorough evaluations of patients to determine their needs and develop effective, individualized treatment plans that are tailored to promote healing and enhance mobility.
• Utilize a wide array of treatment methods, including therapeutic exercises, manual therapy techniques, and modern rehabilitation equipment, to alleviate pain and improve patient function.
• Document and track patient progress meticulously, ensuring clear communication within the healthcare team to modify treatment goals and approaches based on the patient's evolving needs.
• Collaborate with patients, families, and caregivers to provide training and guidance on exercises, use of assistive devices, and implementing therapeutic interventions safely and effectively at home.
• Stay informed about the latest advancements in physical therapy practices through education workshops and professional development opportunities, ensuring your expertise remains cutting-edge.

Patient Population / Client Focus

Your role will involve working closely with a diverse patient population, including children and adults, who are dealing with various diagnoses such as post-operative conditions, orthopedic injuries, neurological disabilities, and chronic pain. By delivering therapy in the comfort of their own homes, you will play a crucial role in helping them regain independence and improve their quality of life in a familiar environment.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid state license to practice physical therapy in New Jersey is essential.
• Excellent verbal and written communication skills to articulate treatment plans and findings to patients, families, and colleagues.
• Strong organizational and technical skills, including proficiency in documentation and familiarity with electronic health record (EHR) systems.
• A flexible and adaptable attitude to navigate the dynamic home care settings.

Preferred Experience

• While a minimum of 2 years of experience is preferred, recent graduates who are eager to learn with mentorship opportunities may also be considered.
• Experience in home care settings and working with a diverse patient population is an advantage.

Compensation and Benefits

This is a competitive full-time position with an annual salary ranging from USD 100,000 to 120,000, commensurate with experience and qualifications.
