Work Environment

This role is an onsite position, placing you directly in the home environment of our clients throughout Newark, New Jersey. This dynamic setting not only allows for personalized interaction with patients but also enables a unique approach to their care—understanding how home dynamics influence their rehabilitation processes. You will collaborate with a talented interdisciplinary team committed to providing coordinated care, encompassing physical therapists, occupational therapists, nursing staff, and case managers.

Key Responsibilities

• Develop and oversee individualized physical therapy treatment plans aimed at restoring functionality and independence, based on thorough assessments and evaluations of each client's physical abilities.
• Evaluate patients' functional abilities through diagnostic tests and assessments, adjusting both treatment approaches and the home environment as necessary to maximize rehabilitation outcomes.
• Deliver a range of therapeutic services including manual therapies, therapeutic exercises, and patient education on device use, ensuring comprehensive support tailored to each individual's lifestyle and needs.
• Collaborate with and supervise physical therapy assistants, ensuring the integration of care aligns with professional standards and state regulations.
• Document patient progress meticulously, incorporating OASIS evaluations, care plans, and discharge summaries to maintain comprehensive clinical records that are submitted on time.
• Conduct regular case conferences, contributing insights and coordinating treatment strategies as an integral member of the interdisciplinary care team.
• Participate in community resource assessments, identifying and recommending services that promote patient independence.
• Provide guidance to patients and family members regarding home safety, rehabilitation techniques, and overall wellness, fostering a supportive environment for recovery.

Patient Population / Client Focus

The clients you will serve primarily consist of individuals recovering from injuries, surgeries, or managing chronic conditions. Our focus is on delivering home care that is patient-centered and tailored to each person's unique circumstances. By evaluating their home setup and mobility requirements, you will be equipped to provide customized strategies and interventions that promote independence and facilitate a positive rehabilitation experience.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current, active Physical Therapist license in the state of New Jersey with a clean history free from sanctions.
• Strong foundational knowledge and practical application of diverse manual therapy techniques.
• Proficiency in creating effective exercise programs that promote rehabilitation and recovery.
• Experience with electronic medical record (EMR) systems, particularly Web PT, for precise documentation.
• Current with evidence-based physical therapy methodologies and trends, ensuring that care aligns with the latest research.

Preferred Experience

• Experience in home healthcare or community-based rehabilitation settings.
• Demonstrated ability to work collaboratively in a team-oriented environment.
• Prior experience training or supervising physical therapy assistants is a plus.
