Position Overview

Perfect Health Physical Therapy Center is seeking a dedicated and compassionate Physical Therapist to join our team on a full-time basis. This role is primarily focused on home care, allowing you the opportunity to provide personalized physical therapy services within the comfort of patients' homes in Woodbridge, New Jersey. As a Physical Therapist, you will evaluate and treat patients, develop customized treatment plans, and collaborate with healthcare professionals to achieve optimal patient outcomes. The annual salary for this position ranges from USD 100,000 to 120,000, in addition to a competitive benefits package.

Work Environment

As part of our commitment to compassionate care, this role requires working onsite in patients' homes. You will be provided with the necessary equipment and support to effectively perform your duties outside of a traditional clinical setting. Key Responsibilities

• Conduct thorough evaluations of patients, inclusive of a review of their medical histories, assessments, and creating comprehensive treatment plans based on findings.
• Communicate evaluation results and treatment plans clearly to patients and their families, involving them actively in their care goals.
• Implement evidence-based treatment protocols tailored to individual patient presentations, adapting interventions as needed based on their progress.
• Collaborate with a multidisciplinary healthcare team, recognizing when to consult specialists or modify care plans based on changing patient conditions.
• Maintain meticulous documentation of patient care activities, including evaluations, progress notes, and discharge summaries, ensuring compliance with all healthcare regulations.
• Manage patient schedules effectively, striving to meet productivity expectations while ensuring timely and efficient service delivery.
• Participate in community outreach and educational activities that promote health and wellness, enhancing access to care and engaging with the wider community.
• Foster a culture of ongoing education by pursuing professional development opportunities and mentoring students or new staff as required.
• Promote a patient-centered experience by demonstrating adaptability to different situations and efficiently navigating barriers to access for the patients we serve.

Patient Population / Client Focus

This position primarily involves working with a diverse patient population, including individuals recovering from surgeries, injuries, or chronic conditions. Our patients range in age and health status, requiring tailored therapy approaches that reflect their unique needs. By working in their homes, you can develop a more profound understanding of their environments and personal circumstances, leading to better, more effective therapy outcomes.

Qualifications

• A Bachelor's Degree in Physical Therapy is required.
• Current and valid Physical Therapy license in the State of New Jersey.
• Basic Life Support (BLS) certification from the American Heart Association for Healthcare Providers is mandatory.
• Strong interpersonal and communication skills to foster relationships with patients and their families.

Preferred Experience

• A minimum of two years of experience as a Physical Therapist is preferred, ideally in a home care or outpatient setting.
• Experience working in a multidisciplinary team environment and familiarity with electronic health records (EHR) is advantageous.
