Position Overview

The Premier Health and Wellness Center is actively seeking a dedicated and experienced Physical Therapist (PT) to join our team in Elizabethport, New Jersey. In this full-time role, you will play an essential part in delivering exceptional patient care in a home care setting, directly contributing to the health and rehabilitation of our patients. Work Environment

This position is onsite within the home care setting, providing services directly to patients in their residences. Your schedule will typically be Monday to Friday, from 8:00 am to 5:00 pm, with occasional flexibility as necessary to accommodate patient needs. Our supportive and dynamic clinical team consists of experienced therapists collaborating with you to ensure optimal patient outcomes.

Key Responsibilities

• Implement individualized physical therapy treatment plans based on patients' specific diagnoses and rehabilitation goals, focusing on improving mobility and functionality.
• Guide patients through therapeutic exercises and activities designed to alleviate pain and enhance physical well-being.
• Educate patients and their families on exercise techniques, body mechanics, and home exercise programs to facilitate independence in recovery.
• Administer therapeutic modalities such as electrical stimulation, ultrasound, and heat/cold therapy while ensuring compliance with prescribed techniques and protocols.
• Document patient progress meticulously, tracking vital metrics such as range of motion, strength improvements, and rehabilitation milestones.
• Prepare for therapy sessions by organizing treatment areas and ensuring that all necessary equipment and supplies are available.
• Engage and communicate effectively with an interdisciplinary team including Physical Therapists, Occupational Therapists, and Speech-Language Pathologists to coordinate integrated patient care.
• Provide emotional support, fostering a nurturing environment that encourages patient motivation and resilience throughout their rehabilitation journeys.
• Adhere to ethical standards while maintaining confidentiality and respecting patient rights.

Patient Population / Client Focus

The Physical Therapist will work with a diverse patient population, including individuals recovering from surgeries, injuries, chronic illnesses, or neurological conditions. Your empathetic approach and therapeutic expertise will enable you to address the unique rehabilitation needs of each patient, enhancing their recovery and quality of life.

Qualifications

• A Bachelor's Degree in Physical Therapy or a related field from an accredited institution.
• A valid state license or certification to practice as a Physical Therapist in New Jersey.
• Strong interpersonal, communication, and organizational skills.
• Capacity to work independently while being a team player within a collaborative environment.

Preferred Experience

• At least 1 year of experience in a physical therapy setting, preferably involving home care or outpatient rehabilitation.
• Hands-on experience in treating a variety of conditions aligned with our specialized services.
• Familiarity with electronic health records (EHR) systems for documenting patient care and progress.

Compensation and Benefits
