Work Environment

This position operates within the home care setting, allowing you to work directly with clients in their own residences. Our office hours are Monday to Friday, from 9 AM to 5 PM, providing a predictable work schedule to help you maintain a healthy work-life balance.

Key Responsibilities

• Perform comprehensive evaluations of patients' physical capabilities and functional limitations to develop tailored treatment strategies.
• Deliver skilled therapy interventions, including therapeutic exercises, manual therapy techniques, and modalities to address the unique needs of our geriatric patients.
• Regularly monitor and document patient progress, using evidence-based methodologies to adapt treatment plans and optimize health outcomes.
• Collaborate with multidisciplinary healthcare teams, including physicians and other therapists, to ensure holistic and integrated care for patients.
• Provide education to patients and their families about their conditions, therapeutic exercises, and strategies for injury prevention.
• Implement safety protocols and fall prevention programs, thereby enhancing patient safety within their living environments.
• Stay abreast of the latest advancements in physical therapy through ongoing education initiatives, ensuring the delivery of best practices.
• Uphold high ethical standards and maintain confidentiality while providing exceptional care.

Patient Population / Client Focus

This role primarily focuses on serving the geriatric population, many of whom face complex health challenges that require comprehensive rehabilitation strategies. Our clients often seek assistance with mobility, pain management, and functional independence. Qualifications

• Possession of a Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
• Active state licensure as a Physical Therapist.

• Proficiency in electronic medical records (EMRs) and documentation systems, facilitating efficient patient care and record management.
• Ability to apply evidence-based practices and the latest therapeutic techniques in treatment planning.

Preferred Experience

• Previous experience in geriatric therapy or home health settings is preferred.
