Work Environment

This position is based in a home care setting, where you will work closely with patients in their residences. This unique environment allows for personalized and direct interactions, making it easier to observe patient behaviors and tailor treatments in real-time. You will be part of an onsite team and collaborate with other healthcare providers to create comprehensive care plans. The work hours are structured from Monday to Friday, 9:00 AM to 5:00 PM, providing a balanced work-life integration.

Key Responsibilities

• Conduct comprehensive assessments of patients' physical conditions, identifying their goals and needs to formulate personalized and adaptive treatment plans.
• Utilize evidence-based practices and a variety of therapeutic techniques to promote recovery, enhance mobility, and decrease pain levels in order to reduce reliance on medications and avoid unnecessary surgical interventions.
• Design and implement specific exercise regimens focusing on strength enhancement, flexibility improvement, balance, and coordination tailored to each patient's requirements.
• Track and document patient progress meticulously, making necessary adjustments to treatment plans based on outcomes and feedback.
• Provide education and resources to patients regarding their conditions, treatment options, and strategies to maintain long-term health and wellness.
• Develop wellness programs that encompass fitness goals aligned with personal patient aspirations and health guidelines.

Patient Population / Client Focus

You will work primarily with patients recovering from orthopedic injuries or surgeries, including but not limited to joint replacements, fractures, and sports-related injuries. Our patient demographic ranges across all ages, from young athletes to older adults seeking to improve mobility and independence. You will have the opportunity to build meaningful relationships with your patients, guiding them throughout their rehabilitation journey.

Qualifications

• Must possess a Master's degree or Doctorate in Physical Therapy from an accredited institution.
• Current and valid state licensure as a Physical Therapist is required.
• Demonstrated experience of at least two years in a clinical setting, specifically in orthopedic rehabilitation.
• Exceptional ability to communicate effectively with patients and healthcare teams, demonstrating strong interpersonal skills.
• An up-to-date understanding of the latest treatment techniques and standards in physical therapy practice.
• Ability to adapt therapies to suit the diverse needs of patients from a variety of cultural backgrounds.

Preferred Experience

Experience in home healthcare settings is preferred, as is familiarity with patient management systems that streamline documentation and treatment tracking. Compensation and Benefits

We offer a competitive annual salary range of $110,000 to $130,000, commensurate with experience and qualifications.
