Work Environment

This position is primarily onsite, focusing on home care, which allows therapists to engage directly with patients in their personal environments. Working with individuals recovering from injuries, diseases, and surgeries, our therapists are equipped with the tools and resources necessary to deliver personalized care. The flexibility of the schedule enables therapists to create their working hours within a Monday to Friday framework, promoting a healthy work-life balance.

Key Responsibilities

In this role, you will be responsible for:

• Evaluating and treating patients in home care settings, crafting individualized therapy plans based on comprehensive assessments.
• Utilizing a variety of therapeutic methods, including exercise programs, stretching techniques, hands-on therapy, and specialized equipment tailored to each patient's needs.
• Documenting treatment plans, progress, and patient responses accurately and timely for ongoing assessments and adjustments to therapy goals.
• Reassessing and redefining therapeutic goals in alignment with patients' progress and needs, ensuring optimal recovery paths.
• Advising patients on the selection and usage of assistive devices to enhance self-care and mobility, improving overall quality of life.
• Training patients, their families, and caregivers in the use of assistive modalities and implementing effective physical therapy procedures.
• Continuously upgrading your professional knowledge through educational workshops, seminars, and networking with peers in the field.

Patient Population / Client Focus

The focus of your role will be on a diverse patient population, ranging from pediatric to geriatric clients. Patients may present with various conditions, including post-surgical recovery, chronic pain management, neurological conditions, and developmental delays. By providing compassionate care tailored to individual needs, you will play a pivotal role in enhancing each patient's recovery journey and overall well-being.

Qualifications

To thrive in this role, candidates should possess:

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid State license as a Physical Therapist.
• Strong written and verbal communication skills to effectively interact with patients and the healthcare team.
• Proficiency in using technology for documentation and communication purposes.
• A flexible and adaptable mindset to meet the varied needs of home care patients.

Preferred Experience

While prior experience is not mandatory, candidates with a minimum of 2 years in a clinical setting are encouraged to apply, as it will enhance the quality of care offered to our patients.
