Work Environment

The Physical Therapist will work in a home care setting, engaging with patients in their residences across Newark and nearby communities. Key Responsibilities

• Conduct comprehensive evaluations that involve reviewing pertinent medical history, performing assessments, and documenting findings to inform treatment plans.
• Effectively communicate evaluation results and treatment options to patients and their families, encouraging their participation in goal-setting and care decisions.
• Implement tailored treatment protocols considering each patient's unique presentation and evolving health status while adhering to best practices in the field.
• Collaborate with a multidisciplinary team of healthcare professionals, identifying any changes in patient conditions that require adjustments to the care plan.
• Ensure thorough discharge planning and follow-up care are executed, meticulously documenting all patient interactions to maintain a clear record of care provided.
• Prioritize effective time management and productivity within a busy schedule, actively participating in both marketing initiatives and departmental goals.
• Engage in continuing education and professional development, contributing to a culture of learning by mentoring students and colleagues.

Patient Population / Client Focus

Our Physical Therapist will primarily engage with a diverse patient population across various demographics, typically providing care to individuals recovering from surgeries, managing chronic conditions, or requiring rehabilitation support following injury. The focus is on delivering empathetic, personalized care that addresses not only the physical but also the emotional and social needs of each patient.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• Current and valid Physical Therapist license in the State of New Jersey.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• Strong interpersonal skills to build rapport with patients and their families, as well as a collaborative spirit to work effectively with fellow healthcare professionals.

Preferred Experience

While prior work experience is not mandatory, a minimum of two years in a home healthcare setting or clinical practice is preferred to ensure the delivery of high-quality care tailored to an evolving patient population. Compensation and Benefits
