Key Responsibilities

• Conduct comprehensive evaluations, gathering pertinent patient history and performing targeted assessments to inform treatment decisions.
• Communicate evaluation findings and collaboratively develop treatment plans with input from patients and their families to establish meaningful recovery goals.
• Exhibit proficient implementation of treatment protocols tailored to each patient's unique presentation and the dynamic nature of healthcare needs.
• Collaborate with other healthcare professionals to ensure a comprehensive care approach, documenting any changes in patient conditions that necessitate updates to the plan of care.
• Complete thorough discharge planning and follow-up care, ensuring documentation reflects all aspects of patient interactions and care progress, including initial evaluations, daily notes, and discharge assessments.
• Manage time effectively, maintaining productivity while ensuring timely attendance to meetings and appointments.
• Engage proactively in team initiatives aimed at removing barriers to access and enhancing service delivery, utilizing relevant data to inform decisions.
• Promote patient-centered care through adaptability in treatment methods and techniques, ensuring a positive experience for each patient.

Patient Population / Client Focus

You will work primarily with a diverse outpatient population, including individuals recovering from surgery, sports injuries, chronic pain, and mobility challenges. Qualifications

• A Doctorate in Physical Therapy (DPT) is mandatory.
• A valid DPT license to practice in the State of New Jersey.

Preferred Experience

Compensation and Benefits
