Key Responsibilities

As a Physical Therapist at Valera Physical Therapy, you will:

• Design and implement personalized treatment plans to prevent disabilities, alleviate pain, enhance mobility, and restore function according to each patient's unique capabilities and physician directives.
• Supervise the efforts of physical therapy assistants to ensure fidelity to care plans and optimal patient outcomes.
• Conduct thorough evaluations of patients' physical condition using diagnostic and prognostic tests, including assessments of muscle strength, nerve function, joint range, and functional abilities.
• Assess home environments to identify modifications that will aid rehabilitation and foster greater independence for patients.
• Administer therapeutic exercises, manual therapies such as massage, and applications of physical agents to effectively manage therapeutic goals.
• Evaluate and fit patients with orthotic and prosthetic devices, providing recommendations for adjustments as needed to aid mobility.
• Document patient progress meticulously, including initial evaluations, care plans, and discharge summaries, ensuring compliance with OASIS requirements for accurate clinical records.
• Engage in multidisciplinary team meetings to contribute to case conferences and collaboratively discuss patient care strategies.
• Educate patients and their families on physical therapy treatments, device usage, and strategies to enhance overall rehabilitation outcomes.
• Support the ongoing professional development of Physical Therapy Assistants by providing guidance in the delivery of patient care as required by state regulations.
• Perform functional assessments and adjust care plans according to patient progress and community resource utilization.

Qualifications

Compensation and Benefits
