Work Environment

This role is primarily performed onsite in the homes of our patients, where you will carry out evaluations and treatment plans in a comfortable and familiar environment for them. As a dedicated Physical Therapist, you will utilize mobile documentation tools and basic therapeutic equipment to facilitate effective treatment sessions. Key Responsibilities

• Conduct comprehensive evaluations of patients to identify impairments and develop tailored treatment plans that align with each patient's specific needs and goals.
• Implement therapeutic interventions that may include exercises, manual therapy, and pain management techniques to promote recovery.
• Monitor patient progress regularly, adjusting treatment plans as necessary to optimize outcomes.
• Ensure complete and accurate documentation of all patient assessments, treatments, and progress notes in accordance with clinic policies.
• Educate patients and their families about their conditions, treatment options, and home exercises to facilitate recovery.
• Maintain compliance with federal and state regulations, including patient rights protocols and universal precautions.
• Participate in ongoing training, team meetings, and performance improvement initiatives to enhance the quality of care.
• Represent Adage Physical Therapy Clinic positively during interactions with patients and external stakeholders.

Patient Population / Client Focus

In this role, you will work predominantly with individuals recovering from surgery, managing chronic conditions, or coping with mobility challenges due to various diagnoses. Our patient population is diverse, encompassing older adults, post-operative patients, and those with disabilities. By providing compassionate and individualized care, you will play an essential role in empowering these patients to regain their independence and improve their overall health.

Qualifications

• A valid and active Physical Therapist license in New Jersey is required.
• Current CPR/BLS certification demonstrating readiness for emergency situations.
• A Bachelor's Degree in Physical Therapy from an accredited institution.

Preferred Experience

• A minimum of one year of clinical experience is preferred, though new graduates with a passion for home health care will be considered.
• Experience with evidence-based physical therapy practices and patient-centric care models will be advantageous.
• Familiarity with electronic medical records (EMR) systems and mobile documentation tools is a plus.
