Work Environment

This full-time position requires you to work onsite, primarily providing home care therapy to patients within the community. You will engage directly with patients in their home environments, allowing you to tailor therapy sessions to their individual lifestyles and needs. Key Responsibilities

• Conduct comprehensive patient evaluations to identify physical impairments and formulate effective treatment plans based on clinical guidelines.
• Implement and modify treatment plans as necessary, measuring patient progress and documenting outcomes meticulously.
• Provide education to patients and their families about therapeutic exercises, injury prevention, and management strategies to enhance recovery.
• Ensure that all patient information is kept confidential and compliant with health information privacy regulations.
• Maintain a clean and safe treatment environment, actively identifying and reporting any hazards.
• Participate in mandatory training sessions and adhere to best practices regarding universal precautions in patient care.

Patient Population / Client Focus

Your primary focus will be on a diverse array of patients requiring rehabilitation due to various conditions, including post-surgical recovery, chronic pain management, sports injuries, and age-related physical decline. By understanding the unique challenges each patient faces in their home environment, you will tailor your approach to facilitate a holistic healing process.

Qualifications

• A current Physical Therapy license in the state of New Jersey is required.
• Certification in CPR/BLS is essential to ensure patient safety during therapy sessions.
• A Bachelor's Degree in Physical Therapy or a related field is necessary for consideration.

Preferred Experience

• At least one year of experience in a clinical setting is preferred; however, new graduates will also be considered based on their educational background and any clinical training.

Compensation and Benefits
