Key Responsibilities

As a Rehabilitation Physical Therapist at Valera Physical Therapy, your responsibilities will encompass a wide array of clinical tasks that are essential for effective patient care:

• Direct and oversee physical therapy treatments focused on alleviating pain, restoring movement, and enhancing overall physical function based on individual assessments.
• Evaluate patient progress regularly using diagnostic methods and functional assessments, adjusting treatment plans as necessary.
• Guide and supervise physical therapy assistants, ensuring the proper implementation of care strategies in line with established protocols and state regulations.
• Conduct thorough home evaluations as needed to determine the environmental factors that may affect rehabilitation and recommend modifications to promote independence.
• Utilize a variety of therapeutic techniques, including manual therapies, exercise programs, and modalities like ultrasound and electrical stimulation.
• Maintain accurate clinical documentation of evaluations, treatment plans, progress notes, and discharge summaries in compliance with agency standards.
• Participate in interdisciplinary team meetings to discuss patient progress and collaborate on integrated care strategies.
• Educate patients and their families about therapy programs, assistive devices, and self-management strategies to promote better health outcomes.
• Lead initiatives to document and track patient outcomes effectively, contributing to the continuous improvement of service delivery.

Patient Population / Client Focus

You will work with a diverse range of individuals, including those recovering from surgeries, injuries, and chronic conditions. Our patient population primarily consists of adults and seniors requiring skilled rehabilitation services to regain independence in their daily activities. Your expertise will be vital in providing targeted interventions that not only address physical rehabilitation but also consider the emotional and psychological aspects of recovery.

Qualifications

The ideal candidate will possess the following qualifications:

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• Current and valid Physical Therapist (PT) license in the state of New Jersey.
• Current certification in Basic Life Support (CPR) and Emergency First Aid.
• Demonstrated strong communication and interpersonal skills, with the ability to engage effectively with patients, families, and team members.
• Proficiency in using electronic medical record (EMR) systems to document patient encounters and maintain compliance with clinical guidelines.

Preferred Experience

Compensation and Benefits
