Full-Time Nurse Practitioner in Cardiology - ProActive Health Solutions

Position Overview

ProActive Health Solutions is looking for a dedicated Nurse Practitioner (NP) to join our team in Elizabeth, New Jersey. This full-time position operates on-site within our Concierge Medicine & House Calls model, focusing on a range of outpatient cardiac care services. Working closely under the guidance of our Chief Nurse Practitioner and the oversight of the Medical Director, this role offers a unique opportunity to engage in comprehensive patient care in a collaborative cardiology practice.

Key Responsibilities

• Collaborate with multiple cardiologists in managing a diverse patient panel, conducting thorough exams and assessments.
• Develop and implement treatment plans for patients with chronic conditions such as hypertension and high cholesterol.
• Provide specialized electrophysiology services to manage patients with arrhythmias, abnormal electrocardiograms, and implanted devices.
• Oversee care for acute cardiac conditions including Coronary Artery Disease, Peripheral Artery Disease, Congestive Heart Failure, and other cardiovascular issues.
• Facilitate and interpret stress tests and other diagnostic procedures as required.
• Utilize clinical judgment to prescribe medications and coordinate referrals to cardiology specialists as needed.
• Engage with patients and family members, delivering education and support to improve health literacy and adherence to treatment plans.
• Maintain accurate and comprehensive electronic health records using our state-of-the-art systems to promote continuity of care.

Patient Population / Client Focus

The Nurse Practitioner will primarily work with adults experiencing a variety of cardiovascular health issues. Patients may present with complex, multi-faceted health concerns requiring a nuanced understanding of cardiology. The focus on concierge care allows for more personalized interactions, leading to improved health outcomes and patient satisfaction. Qualifications

• A Master’s or Doctoral degree from an accredited Nurse Practitioner program.
• Active Nurse Practitioner certification and state licensure to practice in New Jersey.
• Completion of clinical training in cardiology or a related specialty is preferred.
• Strong interpersonal skills and an ability to collaborate effectively in a team-based environment.
• Proficient in electronic health records and clinical documentation practices.

Preferred Experience

• Previous experience in cardiology, especially in outpatient settings, is highly valued.
• Experience with providing education to patients and families regarding cardiac health management.
• Familiarity with stress testing and other cardiology-specific diagnostic procedures.

Compensation and Benefits

We offer a competitive salary range of USD 150,000 to 180,000 annually, commensurate with experience and qualifications.
