Work Environment

Your work setting will specifically involve visiting patients at their homes. Key Responsibilities

• Conduct thorough examinations and evaluations of patients with functional limitations, utilizing a combination of clinical assessments and patient history reviews.
• Develop, implement, and adjust treatment plans based on individual patient evaluations and evolving healthcare practices.
• Maintain meticulous documentation of all patient care activities, including initial evaluations, treatment progress notes, and discharge summaries, ensuring compliance with regulatory standards.
• Collaborate with interdisciplinary healthcare teams and refer patients to appropriate specialists when necessary, documenting all consultations and modifications in care.

Qualifications
