Key Responsibilities\n

• Conduct comprehensive assessments to evaluate patients’ mental health needs and develop tailored treatment plans.

• Facilitate ongoing communication with supervisors, caregivers, and interdisciplinary teams, ensuring cohesive and informed care delivery.

• Deliver a schedule that includes appointments, home visits, and observations to monitor client progress and treatment effectiveness.
\n
• Coordinate home visits to analyze living conditions and gather data for thorough assessments, which may include interviews and chart reviews.
\n
• Implement and supervise adherence to individualized program plans, ensuring they meet the needs of patients.
\n
• Participate actively in Interdisciplinary Team Meetings to collaborate on patient care strategies and program improvements as needed.
\n
• Document patient progress through monthly and quarterly notes, maintaining accurate records for legal and ethical compliance.
\n
• Organize training sessions for team members to enhance their understanding and implementation of treatment protocols.
\n
• Engage in follow-up visits and interviews to provide ongoing support and adjust treatment plans as necessary.
\n
• Act as a liaison with both the public and the broader mental health community to foster support networks and developmental initiatives.

• Assist in creating procedural documents for training materials to continually improve our services and programs.

Patient Population / Client Focus\n

The LPC will primarily serve a diverse clientele, including adults and youth facing various mental health challenges such as anxiety, depression, trauma, and substance use disorders. This role will require you to adapt counseling approaches to meet the unique needs of each individual or family you work with, ensuring support is both effective and culturally competent.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.
\n
• Active and unrestricted licensure as a Professional Counselor (LPC) in Oregon.
\n
• A minimum of two years of counseling experience in clinical settings, showcasing a solid foundation in mental health practices.

• Proficiency in Electronic Medical Records (EMR) and related software for maintaining accurate patient records.

• Strong understanding of mental health legal regulations and ethical standards.

• Proven collaboration skills within healthcare teams, demonstrating the ability to work effectively towards shared goals.

Preferred Experience\n

Compensation and Benefits\n

The salary for this position ranges from USD 90,000 to 100,000 per year, based on your experience and caseload. Additional benefits include:
