Key Responsibilities\n

• Conduct detailed assessments and screenings to assess the mental health status of patients, identifying their needs and developing effective treatment plans tailored specifically to hospice care.

• Provide one-on-one counseling sessions as well as crisis intervention services for patients experiencing acute emotional distress, grief, or anxiety related to end-of-life issues.

• Facilitate therapy groups and educational workshops that cover vital topics, such as coping mechanisms for grief, the emotional impact of terminal diagnoses, and strategies for families supporting loved ones in hospice care.

• Collaborate closely with medical teams, including nurses, doctors, and support staff, to provide a holistic approach to patient care that integrates mental and physical health interventions.

• Engage in community outreach initiatives to raise awareness about mental health issues, emphasizing the importance of emotional support for those facing terminal illnesses.

• Maintain accurate, confidential patient records in accordance with ethical guidelines and regulatory requirements, ensuring a high level of service delivery and accountability.

Patient Population / Client Focus\n

The primary focus of this role is to support hospice patients and their families who are facing significant emotional and psychological challenges. You will provide critical services to individuals who are coping with terminal illnesses, assisting them to navigate their feelings, improve their quality of life, and facilitate a more peaceful end-of-life experience.

Qualifications\n

• Must possess a valid state license as either a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or Licensed Marriage and Family Therapist (LMFT).

• A Master’s or Doctoral degree in Counseling, Psychology, Social Work, or a related field is required.

• Demonstrated expertise in addressing mental health concerns specific to patients in hospice care, with a strong understanding of grief and loss processes.

• Exceptional communication and interpersonal skills necessary to interact sensitively with patients and their families, as well as to collaborate effectively within a healthcare team.

• Familiarity with evidence-based counseling practices and brief therapeutic interventions ideal for a hospice environment.

• Capable of managing a diverse patient caseload in a fast-paced environment while prioritizing tasks to achieve optimal patient outcomes.

Preferred Experience\n
