Key Responsibilities\n

• Conduct and appropriately document thorough, age and culturally sensitive bio-psychosocial assessments to evaluate mental health, substance abuse, and medical needs within a primary care context.

• Deliver personalized behavioral health services to patients, including individual and group counseling, intake assessments, comprehensive evaluations, diagnostic services, treatment planning, and crisis intervention.

• Act as a liaison between patients and community resources, facilitating social work support and providing referrals to local agencies and medication assistance programs tailored to patient needs.
\n
• Collaborate with Quality Improvement teams to design and implement effective processes for data collection, improving service delivery and patient outcomes through evidence-based practices.

• Maintain accurate records, progress notes, and suicide risk assessments in accordance with regulatory requirements and organizational standards.

Patient Population / Client Focus\n

Your role will involve working closely with patients and their families facing end-of-life issues, providing compassionate counseling and psycho-social support tailored to their unique experiences and needs. By utilizing your clinical skills, you will help patients navigate emotional distress, facilitate communication with family members, and create an atmosphere of dignity and respect during their hospice care journey.

Qualifications\n

• A valid and active Licensed Clinical Social Worker (LCSW) license, adhering to California state regulations.

• A Master’s degree in Social Work from an accredited institution.

• At least three (3) years of post-licensure experience in a clinical setting, particularly in hospice or palliative care is preferred.

• Strong knowledge of various counseling techniques, crisis intervention strategies, and the ability to apply independent judgment in therapeutic settings.

• Proficient in group facilitation and planning processes, including managing group dynamics and promoting therapeutic relationships.

Compensation and Benefits\n
