Key Responsibilities\n

• Conduct comprehensive diagnostic assessments and continuously modify individual treatment plans to reflect the evolving needs and conditions of patients.

• Lead therapeutic sessions, offering individual and family counseling to address various mental health challenges, including anxiety, depression, trauma, and behavioral disorders.

• Coordinate care with multidisciplinary teams and community resources, ensuring a comprehensive and seamless approach to treatment that maximizes patient outcomes.

• Implement effective crisis intervention strategies, assessing high-risk situations and providing immediate support or referrals as necessary.

• Maintain accurate documentation of patient interactions and treatment plans in compliance with federal, state, and local regulations, as well as our internal standards.

• Advocate for patients by linking them to necessary community services, support groups, or ancillary healthcare providers to enrich their recovery journey.

• Participate in discharge planning to continue the care process post-treatment, ensuring that follow-up services are arranged and administered correctly.

• Engage in proactive communication with patients, their families, and care teams to address any barriers to treatment and promote a consistent care experience.

• Provide psychoeducation to patients and their families about mental health issues, coping strategies, and available resources.

Qualifications\n

• A Master’s Degree in Social Work or related field from an accredited institution.
