Work Environment\n

This position offers the flexibility of a remote work environment, allowing you to manage your schedule within a full-time framework from 8 AM to 5 PM, Monday through Friday. Our Community Health Clinic integrates technology to facilitate seamless communication and data management, ensuring you can provide high-quality care to clients without geographic limitations.

Key Responsibilities\n

• Determine client needs through detailed assessments, utilizing structured interviews and standardized tools, to develop effective treatment plans tailored to each individual.

• Develop and implement comprehensive support packages, providing direct counseling and crisis intervention while monitoring the effectiveness and progress of treatment.

• Maintain accurate and up-to-date records of client interactions and progress in compliance with state regulations and best practices, ensuring thorough documentation for continuity of care.

• Conduct regular evaluations of client needs, adapting treatment approaches as necessary to align with evolving goals and life circumstances.

• Actively participate in case conferences and collaborate with healthcare professionals, including psychologists, medical staff, and community service organizations, to facilitate comprehensive care.

• Provide education and outreach to clients regarding available community resources, helping them navigate and access social services to better support their recovery journey.

Patient Population / Client Focus\n

Your clientele will include diverse populations facing various social and mental health challenges. You will work with individuals experiencing neglect, trauma, domestic violence, or substance abuse issues. Qualifications\n

• Master's degree in Social Work (MSW) from an accredited institution.
