Work Environment\n

This role will predominantly be performed remotely, allowing for flexible working arrangements while still maintaining a strong connection with clients in the Far Rockaway, New York area. Regular virtual meetings will be held to foster team collaboration, as well as in-person visits when necessary to assess clients' needs directly. Full-time hours are from Monday to Friday, 8:00 AM to 4:30 PM, with potential for occasional evenings and weekends based on client requirements.

Key Responsibilities\n

• Conduct thorough psychosocial assessments of seniors to identify needs and challenges, taking into account emotional, mental, and social factors affecting their well-being.

• Develop and implement personalized care plans in collaboration with seniors and their families, ensuring these plans reflect their individual goals and preferences.

• Provide counseling and emotional support, helping clients navigate life transitions, chronic illness, and other challenges associated with aging.

• Advocate for seniors, facilitating access to community resources and services, such as healthcare, financial assistance, and support groups.
\n
• Coordinate with interdisciplinary teams, including healthcare professionals, to ensure a comprehensive approach to client care.
\n
• Perform regular check-ins through virtual or in-person visits to monitor clients’ progress and address any emerging concerns.

• Facilitate effective communication among family members involved in the care of seniors, addressing conflicts and promoting family dynamics.

• Stay informed about the latest trends in social work practices, ensuring compliance with the ethical standards required for the profession.

• Maintain accurate documentation of client interactions, assessments, and care plans utilizing electronic health records (EHR) to ensure seamless continuity of care.

This role focuses on seniors who may face various challenges associated with aging, including isolation, mental health issues, and the need for social services. The social worker will build meaningful relationships with clients, understanding their unique backgrounds and situations, while empowering them to make informed decisions about their care and social engagement.

Qualifications\n

• A Master's degree in Social Work from an accredited institution, ensuring a robust foundation in social work principles and practices.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory for practice.

Preferred Experience\n

Compensation and Benefits\n
