Work Environment

This position is fully onsite, meaning that as a Home Care Physical Therapist, you will travel to patients' homes within the Metuchen area. You will work a consistent Monday to Friday schedule, averaging 40 hours per week. Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians, establishing clear baselines for treatment goals and expectations.
• Design and implement individualized treatment plans based on assessment outcomes, focusing on a variety of specialties including orthopedic, pediatric, neurologic, and sports medicine.
• Maintain excellent communication with patients and their families, demonstrating outstanding customer service while managing their expectations and treatment journey.
• Collaborate with an interdisciplinary team to optimize patient outcomes, utilizing evidence-based practices and innovative methodologies.
• Deliver therapeutic care in-home using advanced techniques tailored to the patient's specific needs, ensuring adherence to the physician's plan of care.
• Provide education and training to patients and their caregivers, empowering them with knowledge and skills to facilitate recovery and prevent further injuries.
• Achieve minimum utilization targets and other business performance goals to contribute to the sustainability and growth of our practice.

Patient Population / Client Focus

In this role, you will engage with a diverse demographic of patients, ranging from pediatric to geriatric populations. Your approach will cater to individuals recovering from surgeries, injuries, or chronic conditions requiring rehabilitation. By customizing your methods to each patient's unique situation and needs, you will help them overcome challenges and reach their functional goals.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Active state licensure as a Doctor of Physical Therapy through the Physical Therapy Examining Committee.
• A minimum of two (2) years of relevant experience in a clinical setting.
• Current CPR certification to ensure patient safety.
• Strong verbal and written communication skills essential for effective patient interactions.
• Basic proficiency in word processing and computer technology.
• A collaborative mindset with a desire to establish enduring relationships with patients and their families.

Preferred Experience

• Experience working specifically in home care settings is preferred but not mandatory.
• Prior involvement with evidence-based practice methodologies in a clinical context.
• Strong ability to build rapport and trust with diverse patient demographics.

Compensation and Benefits
