This position is based onsite at our state-of-the-art Rehabilitation Center located in Freehold, New Jersey. Key Responsibilities

• Conduct thorough evaluations of patients to assess their physical condition and rehabilitation needs in a home care setting.
• Develop personalized treatment plans that utilize evidence-based practices, including therapeutic exercises, manual therapy, modalities, and patient education.
• Implement treatment plans to assist patients in recovering from injuries, surgeries, or chronic conditions, focusing on pain reduction and mobility enhancement.
• Track and maintain detailed documentation of patient progress, responses to treatment, and modifications to the treatment plan.
• Reassess patient conditions periodically, adjusting treatment objectives and strategies as progress is made or needs change.
• Guide patients and their families in the use of assistive devices and provide training on effective self-care techniques.
• Participate in inter-disciplinary team meetings to ensure a coordinated approach to patient care, sharing insights and contributing to overall treatment goals.
• Stay current with advancements in physical therapy practices through attendance at workshops, seminars, and professional networking opportunities.

Patient Population / Client Focus

You will be working with a diverse clientele, which includes children, adults, and the elderly, each presenting a unique set of rehabilitation needs. Our patient population encompasses those recovering from surgery, acute injuries, or chronic pain conditions. By focusing on an individualized approach, you will help patients achieve their specific therapeutic goals, enhancing their functional abilities and quality of life.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution; a Doctorate in Physical Therapy (DPT) is highly preferred.
• Active State License in Physical Therapy, ensuring compliance with local healthcare regulations.
• Strong verbal and written communication skills, enabling effective interaction with patients, families, and other healthcare professionals.
• Proficient in documentation and use of electronic health records (EHR) systems.
• Ability to work both independently and collaboratively within a team, demonstrating adaptability to varying patient needs and environments.

Preferred Experience

• Minimum of 2 years of experience in a clinical physical therapy setting.
• Experience in home care is a plus but not mandatory; a willingness to learn and develop new skills is essential.

Compensation and Benefits
