Key Responsibilities

• Conduct comprehensive evaluations of patients by reviewing medical history, performing physical assessments, and establishing personalized treatment plans tailored to individual needs.
• Clearly communicate evaluation results and treatment strategies to patients and their families, ensuring their involvement in the goal-setting process.
• Implement evidence-based treatment protocols and demonstrate flexibility in adapting methods based on evolving patient conditions and feedback.
• Collaborate with interdisciplinary healthcare teams to optimize patient care and document any necessary adjustments to treatment plans based on observed changes in patient progress.
• Ensure accurate and timely documentation of patient care activities, including detailed encounter notes, progress summaries, and discharge evaluations, fostering a transparent record of the patient's rehabilitation journey.
• Participate in community outreach and marketing activities to promote the organization's mission and services, actively contributing to a positive community image.
• Support departmental goals by engaging in ongoing professional development, serving as a mentor to students and new staff members.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, including individuals recovering from surgery, injuries, and chronic conditions. Qualifications

• Certification in Basic Life Support (BLS) for Healthcare Providers as mandated by the American Heart Association.
