Key Responsibilities\n

• Conduct thorough evaluations and assessments to identify each child's unique strengths and areas for growth.

• Play an active role in the development and implementation of Individualized Family Service Plans (IFSP), working closely with families to ensure alignment with their goals.

• Monitor and assess outcomes collaboratively with a multidisciplinary team, ensuring that every child's developmental milestones are met.

• Offer direct support and guidance to families, fostering understanding and effective strategies that enhance their child's development.

• Utilize the coaching model and practices that integrate learning in natural environments for improved outcomes.

• Adhere to all local and national regulations to maintain a safe and compliant practice environment.

Patient Population / Client Focus\n

This role primarily focuses on infants and toddlers diagnosed with developmental delays and disabilities. Your interactions will involve supporting families from various backgrounds, empowering them to engage in their child's learning and growth through tailored strategies that promote physical, cognitive, social/emotional, and communication skills.

Qualifications\n

• Master's Degree or Doctorate of Social Work from an accredited institution.

• Current licensure as an LCSW in the state of Connecticut, issued by the Department of Public Health.

• A valid driver's license is required for traveling to client homes when necessary.

Compensation and Benefits\n
