Key Responsibilities\n

• Engage directly with clients experiencing symptoms of major mental illnesses and severe emotional disorders, providing clinical support through telehealth and in-person meetings as needed.

• Develop individualized care plans that are integrated and focused on enhancing clients' access to linked services, facilitating communication amongst clients, families, and healthcare providers.

• Conduct developmentally appropriate screenings and assessments or arrange for comprehensive evaluations when necessary.

• Lead and participate in multi-disciplinary team meetings to discuss client progress and re-evaluate treatment strategies to ensure optimal outcomes.

• Maintain case documentation and ensure all client interactions and interventions are accurately recorded in health management systems.

• Identify and report any potential safety risks, ensuring a secure and supportive environment for clients.

• Foster a supportive atmosphere that values collaboration, trust, diversity, and empowerment in both client relationships and colleague interactions.

Qualifications\n

Compensation and Benefits\n
