Key Responsibilities\n

• Conduct thorough mental health assessments via telehealth sessions to identify patients' psychological needs and formulate individualized treatment plans.

• Utilize evidence-based therapeutic methods, including cognitive-behavioral therapy (CBT) and mindfulness techniques, to deliver effective mental health interventions to patients and their families.

• Facilitate virtual support groups that provide platforms for patients and families to share experiences and coping strategies, fostering a supportive community atmosphere.

• Collaborate with a variety of healthcare professionals, including physicians, nurses, and social workers, to ensure a unified and holistic approach to patient care.

• Provide crisis intervention services for patients experiencing acute psychological distress, including suicide risk assessments and emergency response strategies.

• Inform patients and their families about available mental health resources, coping strategies, and community services, ensuring they are empowered in their care journey.

Patient Population / Client Focus\n

As part of our hospice team, you will primarily work with patients facing terminal illnesses and their families. Qualifications\n

• A Bachelor's degree in Social Work, Psychology, Counseling, or a related field.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or similar in the state of New York.

Compensation and Benefits\n
