Key Responsibilities\n

• Conduct ongoing assessments of patients to identify their mental health needs, implementing care plans tailored to individual goals.

• Maintain open lines of communication with supervisors, care staff, family members, and other relevant stakeholders to facilitate a comprehensive understanding of patient progress.

• Develop and share a tentative schedule of appointments, home visits, and therapeutic observations to ensure consistent monitoring of patient advancements.

• Coordinate and perform home visits for in-home observations, assisting with chart reviews, conducting staff interviews, and analyzing pertinent data.

• Ensure the effective implementation of treatment plans and modify them in collaboration with the interdisciplinary team based on patients' progress.

• Participate in interdisciplinary team meetings and various case discussions to streamline patient care and support processes.
\n
• Document and complete monthly and quarterly progress notes to track patient development and service efficacy.
\n
• Organize and conduct training sessions for staff regarding the proper implementation of treatment plans and documentation techniques.

• Provide follow-up interviews and home visits to ensure the continuity of care and commitment to addressing patient needs.

• Maintain organized patient records and data to guarantee accessibility for healthcare teams and stakeholders.

• Act as a liaison with public and private mental health resources to enhance community support and development goals.
