Key Responsibilities\n

• Guide patients in accessing community resources, including support groups and vocational rehabilitation services, to aid in their recovery journey.

• Educate patients and families about post-rehabilitation services, ensuring a smooth transition and continuity of care.

• Play a pivotal role in discharge planning, facilitating safe transitions to home or other care settings.

• Regularly document patient progress and treatment outcomes in electronic health records, ensuring adherence to best practices and regulatory standards.

Patient Population / Client Focus\n

The primary focus of this role will be on individuals receiving rehabilitation services in hospice care, which may include those recovering from strokes, spinal cord injuries, orthopedic surgeries, and other neurological disorders. Your interventions will be crucial in addressing both the mental and emotional aspects associated with long-term recovery, allowing patients to navigate their rehabilitation journeys effectively.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

Compensation and Benefits\n

We offer a competitive annual salary in the range of USD 80,000 to 100,000, complemented by a sign-on bonus.
