Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients’ mental health needs, identifying their strengths and challenges to inform tailored treatment strategies.

• Provide individual, family, and group therapy sessions, using evidence-based practices to address unique situations, particularly those encountered in hospice care.

• Utilize therapeutic modalities such as cognitive-behavioral therapy, motivational interviewing, and family systems therapy to enhance mental health outcomes.

• Develop personalized treatment plans that outline specific goals, interventions, and metrics for monitoring progress, adjusting as necessary to meet clients' evolving needs.

• Work collaboratively with a multidisciplinary team, including psychiatrists, nurses, and other healthcare professionals, to coordinate holistic care that addresses both psychological and physical well-being.

• Deliver crisis intervention services, executing risk assessments and safety planning to protect clients during critical moments.

• Act as an advocate for clients, connecting them with essential community resources, and providing thorough case management to support their overall well-being.

• Engage in community outreach initiatives to foster mental health awareness and reduce stigma associated with seeking mental health care.

Patient Population / Client Focus\n

This position will involve working with a diverse population of individuals and families affected by serious illness and end-of-life issues. Clients may be experiencing a range of emotions including grief, anxiety, and depression, and will require a compassionate and skilled social worker to help them navigate these complex feelings. Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
