Key Responsibilities\n

• Deliver high-quality counseling and psychotherapy interventions to patients, utilizing evidence-based treatment modalities tailored to individual needs.

• Create personalized treatment plans in collaboration with clients, setting measurable goals that track progress throughout the therapeutic journey.

• Act as an advocate for clients by linking them with essential resources such as financial assistance, housing options, and additional social services, ensuring holistic support.

• Provide immediate crisis intervention for clients facing acute mental health issues, integrating strategies to manage suicidal ideation, substance use crises, or traumatic responses.

• Engage in community-based initiatives and educational programs aimed at promoting mental health awareness and reducing stigma associated with mental health challenges.

• Maintain meticulous case records, treatment plans, and progress notes in compliance with HIPAA regulations and organizational standards.

Patient Population / Client Focus\n

As part of our hospice-centered approach, you will primarily work with clients experiencing the psychological and emotional challenges that accompany terminal illness, grief, and loss. Your diverse client base will include individuals coping with chronic conditions, their families, and caregivers, all needing empathetic support and guidance through their unique journeys.

Qualifications\n

• Master's degree in Social Work from an accredited program.

• Valid Virginia LCSW license with the ability to practice independently.

Compensation and Benefits\n
