You will be part of a supportive hospice center environment, focusing on mental health alongside physical health, ensuring holistic care for clients who may be facing end-of-life issues or chronic conditions.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments, identifying clients' mental health issues, strengths, and available resources to inform treatment plans.

• Provide individual, family, and group therapy with techniques rooted in evidence-based practices such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed approaches.

• Collaborate closely with psychiatrists and other professionals to secure a cohesive approach to client care.
\n
• Develop and implement individualized treatment plans, outlining clear therapeutic goals and actionable interventions tailored to client needs.

• Facilitate engaging group therapy sessions and psychoeducation classes, helping participants build skills in a supportive environment.

• Utilize crisis intervention techniques and conduct risk assessments to ensure client safety and well-being.

• Maintain detailed and accurate documentation of therapeutic interventions in compliance with ethical and legal standards.

Patient Population / Client Focus\n

In this role, you will engage with a diverse clientele, including adults and older adults dealing with moderate to severe mental health issues. Many of our clients are navigating significant life transitions, including grief and loss, chronic illness, and other factors that impact their mental health. Your interventions will be critical in helping them find pathways to healing and stability.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years’ experience in a mental health or outpatient practice setting.

• Solid understanding of evidence-based therapies and treatment modalities.

• Exceptional interpersonal, communication, and assessment skills that foster client trust and rapport.

Compensation and Benefits\n
