This position will be conducted remotely, providing flexibility in your work-life balance while maintaining accountability and professionalism in all interactions. While you will primarily work from home, you will also have the opportunity to collaborate with healthcare team members and engage in regular virtual meetings to discuss treatment plans and patient progress.

Key Responsibilities\n

The Licensed Clinical Social Worker will be responsible for a variety of essential tasks that directly contribute to the treatment and recovery of clients. Key responsibilities include:

• Actively communicating with physicians and other healthcare providers to report on patient progress and concerns, ensuring coordinated care.

• Maintaining comprehensive, accurate, and timely clinical records and progress notes in accordance with best practice standards and department policies.

• Working collaboratively with the healthcare team to develop individualized treatment plans and discharge plans based on thorough psychosocial assessments.

• Providing effective counseling and crisis intervention services, addressing emotional, behavioral, and psychological challenges faced by clients.

• Assisting clients with discharge planning and care coordination, liaising with various service providers to facilitate access to needed resources.

• Consulting with family members and support systems to enhance understanding of client treatment plans and discharge strategies.

Patient Population / Client Focus\n

This role primarily serves an outpatient population experiencing a range of mental health challenges, including anxiety, depression, and trauma-related disorders. Clients might include adults, adolescents, and families seeking assistance in navigating their mental health journeys. As a mental health counselor, you will provide tailored support to empower clients to achieve their treatment goals.

Qualifications\n

• A Master’s degree in Social Work or Counseling from an accredited program.

• At least 2 years of professional experience working in an outpatient mental health setting.

• Proficiency in computer applications such as MS Office Suite and familiarity with electronic medical records (EMR) systems.

Compensation and Benefits\n
