Key Responsibilities\n

• Engage with patients and their families to assess behavioral health needs and develop treatment plans that leverage community resources.

• Educate patients and families about their discharge options and community services available based on mental health, legal, and insurance circumstances.

• Coordinate follow-up appointments with psychiatric providers within 7 days of discharge, ensuring a seamless transition to a higher level of care.

• Conduct thorough safety planning for patients and families, including risk assessments and interventions such as gun removal when necessary.

• Collaborate with a multidisciplinary healthcare team, including court officers and patient advocates, to facilitate patient progress while respecting patient rights and maintaining facility safety.

Compensation and Benefits\n

We offer a competitive salary range of USD 120,000 to 130,000 annually, reflecting your skills and experience.
