Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, evaluating emotional, mental, and social factors to develop informed care plans.

• Collaborate closely with clients and their families to create customized care strategies that reflect individual goals and values.

• Provide counseling and emotional support, addressing life transitions such as health changes, end-of-life issues, and family dynamics.

• Advocate for clients, ensuring they have access to necessary community resources, including healthcare, financial assistance, and support groups.

• Engage with interdisciplinary teams—nurses, caregivers, and healthcare professionals—to coordinate care effectively, enhancing service delivery and client outcomes.

• Regularly visit clients to monitor their well-being and safety, address emerging concerns, and make adjustments to care plans as needed.

• Facilitate open communication among family members to resolve conflicts and create cohesive support systems for seniors.

• Educate clients and families about available community resources, empowering them to engage with local programs that foster socialization and improve overall quality of life.

• Document all client interactions, assessments, and progress notes meticulously to ensure continuity of care and compliance with ethical standards.

Qualifications\n

Compensation and Benefits\n
