Key Responsibilities\n

In your role as an LPC at Crestwood Futures Counseling, your day-to-day responsibilities will include:

• Conducting comprehensive assessments to ascertain the unique needs of each patient you serve, ensuring individualized therapeutic interventions.

• Facilitating clear and effective communication among supervisors, direct care staff, families, and other stakeholders regarding each patient’s progress.

• Providing a structured schedule to supervisors that includes appointments, home visits, and observations for continuous work advancement.

• Coordinating in-home visits and assessments that may require chart reviews and collaboration with other health professionals.

• Overseeing the execution of the treatment plan to ensure it is practical and effective, and adjusting as necessary based on patient feedback or progress.

• Participating actively in Interdisciplinary Team Meetings, contributing valuable insights into strategies for patient engagement and improvement.

• Documenting patient progress through monthly and quarterly reports, maintaining accurate records to support ongoing care.

• Organizing training sessions to ensure all team members are well-versed in the implementation of treatment plans and documentation processes.

• Conducting follow-up assessments and home visits that reinforce therapeutic support and address any emergent patient needs.

• Acting as a liaison between the public, mental health community, and our organization, advocating for patient access to appropriate resources.

Qualifications\n

The ideal candidate for this position should possess the following qualifications:

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is mandatory.

• An active and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona.

Compensation and Benefits\n
