This position is situated within a community health clinic setting, offering remote work flexibility. Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to determine the developmental needs of children.

• Actively collaborate in the development and implementation of Individualized Family Service Plans (IFSPs).

• Monitor and assess outcomes, providing ongoing support as a valued member of interdisciplinary teams.

• Offer evidence-based recommendations and strategies tailored to each child’s specific learning style and requirements.

• Utilize the coaching model to teach families how to promote their child's development within natural environments.

• Provide families with education and resources to assist with their child's developmental goals and support needs.

• Adhere to all federal and state regulations, ensuring compliance and safety for all individuals served.

Patient Population / Client Focus\n

The LCSW role primarily serves children from birth to three years old who may have developmental delays or disabilities. Our focus is on empowering families to support their children’s early growth and learning, employing strategies informed by natural learning environment practices. Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• Current licensure from the Department of Public Health as a Licensed Clinical Social Worker (LCSW).
