Work Environment\n

The Licensed Clinical Social Worker position is structured as a remote role, allowing for flexibility while maintaining a strong connection to our hospice center’s values and mission. You will engage with clients primarily via telehealth platforms, with the occasional requirement to participate in in-person meetings or community outreach as needed. Our hours of operation are Monday through Friday from 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend hours depending on client needs and availability.

Key Responsibilities\n

• Provide clinical assessments and individualized treatment plans for clients, including conducting initial evaluations and ongoing progress assessments to address symptoms of major mental illness or severe emotional disorders.

• Engage in teletherapy sessions with clients, utilizing evidence-based practices to support their mental and emotional well-being in a hospice setting.

• Collaborate effectively with interdisciplinary teams, including medical professionals and caregivers, ensuring continuity of care and integrated service delivery.

• Develop and facilitate support groups or therapeutic interventions tailored to the unique needs of clients experiencing end-of-life transitions.

• Navigate complex organizational and financial systems, advocating for client resources and adhering to the ethical guidelines and policies established by Polaris Behavioral Health Center.

• Maintain accurate documentation in compliance with healthcare regulations, ensuring a clear communication trail regarding client progress and treatment strategies.

• Continuously improve clinical skills through active participation in training, team meetings, and performance evaluation sessions to enhance service delivery and client outcomes.

Patient Population / Client Focus\n

This role primarily serves clients in hospice care, who may be coping with severe emotional distress related to terminal illness and many life transitions. You will provide empathetic support that acknowledges their unique life stories, cultural backgrounds, and personal struggles. Personalized care plans will focus on choice and empowerment, and you will be instrumental in addressing both the clients’ and their families' emotional needs during this sensitive time.
