Work Environment

This full-time role operates in an onsite setting as part of our home care facility, where you will interact not only with patients but also a diverse group of healthcare professionals to ensure comprehensive treatment approaches. Your work schedule will be from Monday to Friday, 8:00 am to 5:00 pm, allowing for a balanced work-life arrangement. Our clinic is designed to promote patient comfort and recovery, and you will be surrounded by state-of-the-art equipment and therapeutic tools that support a range of rehabilitation techniques.

Key Responsibilities

• Conduct evaluations and assessments, establishing a baseline for each patient's functional abilities while developing personalized treatment plans tailored to individual needs.
• Implement evidence-based treatment protocols based on evaluation outcomes, ensuring alignment with patients' rehabilitation goals and adapting as necessary to changes in their condition.
• Document patient progress meticulously, including detailed initial evaluations, daily notes, interim progress reports, and comprehensive discharge summaries to facilitate continued care.
• Collaborate with a multidisciplinary team of healthcare professionals, including physicians, occupational therapists, and nursing staff, to address complex patient cases effectively.
• Stay updated on the latest treatment modalities and best practices, actively participating in facility meetings and team initiatives to eliminate barriers to patient access.
• Assist in planning for patient discharge and follow-up care, ensuring a seamless transition and continued support for improved health outcomes.
• Maintain compliance with all local, state, and federal regulations, including participation in company-sponsored trainings for ongoing professional development.

Patient Population / Client Focus

The primary patient population includes individuals undergoing rehabilitation for orthopedic injuries, sports-related trauma, and post-operative recovery. You will work closely with patients to enhance their mobility, reduce pain, and empower them towards functional independence. Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited institution is required for this role.
• State licensure as a Physical Therapist (PT) is mandatory.
• Current CPR certification is essential to ensure patient safety.

Preferred Experience

• A minimum of two years' experience in outpatient orthopedics, providing hands-on clinical treatment, is preferred.
• Experience in working collaboratively within a team and engaging with patients from diverse backgrounds is highly valued.
