This role is set within an outpatient office environment, facilitating a hybrid telehealth and in-person service model. You will have the ability to conduct remote therapy sessions, manage appointments, and coordinate care while also being part of a supportive team that prioritizes collaborative approaches to mental health care. Key Responsibilities\n

• Conduct Comprehensive Assessments: Regularly assess the needs of clients to formulate effective treatment plans, tailoring strategies to individual circumstances.

• Collaborative Communication: Serve as a key point of contact among supervisors, direct care staff, families, and other stakeholders, updating them on clients' progress and treatment adjustments.

• Monitor Progress: Deliver clear updates of appointment schedules, home visits, and observations to supervisors and maintain organized records for transparency.

• Home Visit Coordination: Plan and execute home visits to provide in-home support, conducting observations and revisiting treatment goals as necessary.
\n
• Program Implementation: Ensure that the treatment plan is being followed accurately, making necessary adjustments based on client needs and feedback.
\n
• Engagement in Team Meetings: Actively participate in interdisciplinary team meetings to discuss patient welfare and contribute insights from your counseling expertise.

• Documentation: Complete essential monthly and quarterly progress notes while maintaining thorough records that are easily accessible to relevant parties.
\n
• Training Facilitation: Organize and lead training sessions aimed at ensuring staff members are fully equipped to implement treatment plans and documentation practices.

• Client Follow-Up: Conduct follow-up interviews and home visits as part of the ongoing support system for clients post-treatment.

• Community Liaison: Represent the organization within the mental health community, collaborating with external stakeholders to foster a supportive environment for clients’ treatment goals.

• Development and Training Resource Creation: Assist in creating training materials designed to improve the competencies of coaching programs for clients.

Patient Population / Client Focus\n

Your work as an LPC will involve directly supporting a diverse population that includes both adults and youths. This role allows for impactful engagements with individuals facing challenges such as emotional distress, trauma, substance use disorders, and family dynamics. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in New Jersey.
