Key Responsibilities\n

• Conduct in-depth assessments to develop individualized treatment plans tailored to the unique needs of each client, ensuring effective interventions are in place.

• Actively engage in communication with supervisors, healthcare team members, and family members to facilitate comprehensive care and monitor client progress.

• Provide a structured schedule outlining appointments, home visits, and other relevant observations for better tracking of client development.
\n
• Coordinate and conduct home visits for in-home observations, chart reviews, and data analysis to ensure holistic client care.
\n
• Oversee the implementation of treatment plans, making adjustments as necessary based on feedback and progress updates.
\n
• Participate in Interdisciplinary Team Meetings, collaborating with peers to share insights and strategies for patient care.
\n
• Regularly complete and update monthly and quarterly progress notes, documenting client advancements and areas for continued focus.
\n
• Organize and facilitate training for staff and team members, ensuring they are proficient in implementing treatment plans and maintaining proper documentation.

• Maintain up-to-date records that are accessible to relevant parties, ensuring confidentiality and compliance with legal standards.

• Serve as a liaison with the community and mental health organizations, working towards shared goals of development and support.

Patient Population / Client Focus\n

This role primarily involves working with a diverse client population, including adults and youth facing a variety of mental health issues, such as anxiety, depression, trauma, and substance misuse. By understanding the unique challenges faced by different demographics, you will provide targeted support and engage clients in their journey towards recovery.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Colorado.

• A minimum of 2 years of counseling experience in a clinical setting is required.

• Strong preference for candidates with specialization in family therapy, couples therapy, trauma-informed care, or substance misuse treatment.

• Demonstrated understanding of legal and ethical counseling standards.
\n
• Exceptional interpersonal and communication skills, enabling rapport-building with clients from diverse backgrounds.

• Proven ability to collaborate effectively within a healthcare team to provide comprehensive client care.

• Familiarity with Electronic Medical Record (EMR) systems and relevant technological tools for maintaining accurate client information.
