Key Responsibilities\n

• Conduct thorough psychosocial assessments and develop individualized treatment plans in collaboration with the healthcare team.

• Communicate effectively with physicians and other healthcare professionals to relay patient progress, needs, and concerns, ensuring a cohesive treatment approach.

• Maintain meticulous and timely clinical documentation, including progress notes and case summaries, adhering to organizational policies and best practices.

• Provide counseling and crisis intervention to clients, addressing a range of emotional and behavioral issues through evidence-based therapeutic techniques.

• Assist clients in discharge planning, coordinating care with various service providers to facilitate smooth transitions and continued support.

• Engage with clients’ support systems and family members to discuss treatment, progress, and needs to empower a holistic recovery process.

Patient Population / Client Focus\n

As a Licensed Professional Counselor at Pathways Wellness Center, you will serve a diverse clientele facing various mental health challenges, including depression, anxiety, trauma, and coping difficulties. Your role involves creating supportive environments where patients feel safe to express their thoughts and emotions. You will work closely with individuals from different backgrounds, tailoring your counseling strategies to meet their unique needs and enhancing their overall well-being.

Qualifications\n

• A Master’s degree in Professional Counseling from an accredited institution.

• Current and valid licensure as a Licensed Professional Counselor (LPC) or equivalent.

• A minimum of two years of experience in providing direct mental health services, including both individual and group counseling.

• Self-directed and organized with the ability to manage a caseload effectively.

• Proficient in using technology for telehealth services and familiar with electronic health records systems, as well as standard office software.
