Key Responsibilities\n

As a Senior Licensed Clinical Social Worker, you will perform a variety of essential tasks:

• Collaborate closely with clients and their families to create and implement personalized care plans that reflect their preferences and needs.

• Advocate for clients’ rights, facilitating access to community resources and ensuring they receive necessary support services.

• Maintain communication with interdisciplinary teams, including healthcare professionals, to coordinate care effectively and enhance patient outcomes.

• Monitor clients' conditions through regular virtual visits, addressing safety concerns and any emerging issues that may impact their quality of life.

• Educate clients and families about community resources, support groups, and programs intended to enhance social engagement.

• Accurately document client interactions, assessments, and treatment plans in compliance with regulatory standards and organizational policies.

Patient Population / Client Focus\n

Your primary focus will be on seniors in the hospice setting, many of whom face significant emotional and psychological challenges. By addressing their unique needs, you will not only provide immediate support but also empower seniors and their families to navigate complex decisions related to end-of-life care.

Qualifications\n

• A Master's Degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of relevant experience in a social work capacity, ideally within geriatric care or hospice settings.

Compensation and Benefits\n
