Key Responsibilities\n

• Conduct thorough psychosocial assessments to understand clients' mental health challenges and their personal strengths, leading to effective treatment planning.

• Deliver individual, family, and group therapy sessions employing advanced evidence-based practices such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed care.

• Design and implement personalized treatment plans that include specific therapeutic goals and tailored interventions based on each client's unique situation.

• Engage in collaborative care by partnering with interdisciplinary teams to ensure a cohesive approach to each client’s treatment, ensuring all dimensions of care are considered.

• Provide crisis intervention and safety planning when necessary, demonstrating the ability to respond effectively to urgent client needs.

• Maintain comprehensive and accurate documentation compliant with ethical standards and legal requirements, ensuring continuity of care.

• Facilitate support groups, psychoeducation classes, and therapeutic group sessions that promote healing within our client community.

Patient Population / Client Focus\n

Your role will primarily focus on clients coping with end-of-life issues, including those facing terminal illnesses and their families. You will provide the emotional support necessary to help these individuals process grief, fear, and anxiety, while also offering practical guidance and resources to foster resilience during challenging times.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).
\n
• A minimum of two years’ experience in mental health settings, specifically with knowledge of hospice or palliative care.

• Strong background in various evidence-based therapeutic modalities, allowing you to respond to diverse client needs effectively.

• Exceptional interpersonal and communication skills to establish trust and rapport with clients and team members.

Preferred Experience\n

• Previous experience working in hospice or palliative care environments is highly preferred.

Compensation and Benefits\n
