Key Responsibilities\n

• Conduct in-depth assessments and evaluations to identify clients’ mental health needs, customizing treatment plans accordingly.

• Facilitate effective communication among care teams, family members, and clients to collaboratively discuss progress and adjustments to treatment plans.

• Organize and maintain a structured timetable for appointments, incorporating home visits and teletherapy sessions to monitor client progress.

• Develop comprehensive documentation of client interactions and progress to meet both care standards and legal requirements.

• Participate in Interdisciplinary Team Meetings, contributing insights into program implementations and client developments.

• Provide ongoing support through follow-up interviews and virtual home visits to address clients' evolving needs.

• Prepare and present monthly and quarterly reports on client outcomes and program effectiveness to ensure accountability and transparency.

• Engage in continuous professional development, including coordinating and leading training sessions to enhance the skills and knowledge of fellow staff members.

• Maintain an extensive resource repository to ensure clients and their families have access to valuable support materials and tools.

• Act as an advocate and liaison between clients and the mental health community, assisting clients in achieving their therapeutic goals.
